چکیده

Widespread intracardiac surgery of rheumatic heart disease has brought to medical attention a puzzling postoperative complication, usually designated the "posteommissurotomy syndrome" and often considered to represent reactivation of rheumatic fever. Herein are reported instances of an identical complication following intrapericardial surgery of congenital heart disease. It is suggested that the delayed febrile pleuropericardial reaction represents a traumatic pericarditis and that a preferable name for it is the postpericardiotomy syndrome.

Constrictive pericarditis after surgical closure of atrial septal defect in a child.

Constrictive pericarditis following an open cardiac surgery for congenital heart disease is very rarely encountered in children. We report a case of constrictive pericarditis, diagnosed at 22 months after surgical closure of an atrial septal defect, in a 14-year-old boy who presented postpericardiotomy syndrome. Cardiac catheterization revealed uniformity of elevated diastolic pressures in all ...

Chronic Postpericardiotomy Syndrome and Cardiac Tamponade Lasting for Two Years after Open Heart Surgery

Postpericardiotomy syndrome (PPS) is a major cause of morbidity after cardiac surgery and may cause bypass graft closure and fatal cardiac tamponade. Treatment modalities include nonsteroidal antiinflammatory drugs, corticosteroids, diuresis, drainage, and pericardiectomy in severe cases. Usually the syndrome lasts weeks only, and persistence beyond six months is exceptional. We describe a 64-y...

Late cardiac tamponade after open heart surgery: incidence, role of anticoagulants in its pathogenesis and its relationship to the postpericardiotomy syndrome.

SUMMARY Cardiac tamponade that occurs late after cardiac surgery (7 days) is relatively uncommon but potentially fatal. We analyzed its incidence, clinical course and relationship to the postpericardiotomy syndrome in 1290 consecutive adult patients who survived surgery. Criteria for diagnosis of cardiac tamponade were (1) elevated jugular venous pressure, (2) hypotension or decreased cardiac i...

The Postpericardiotomy syndrome and antiheart antibodies.

In a prospective, double-blind study, heart-reactive antibody in high titer was detected, using an indirect immunofluorescent technique, in the serum of patients in whom the postpericardiotomy syndrome developed after intrapericardial surgery. The syndrome occurred in 26 of 86 consecutive longterm survivors of such surgery, an incidence of 30%. Presence of antibody correlated closely with the c...

Rheumatic symptoms after cardiac surgery: a prospective study.

The incidence of different types of shoulder pain after open heart surgery was studied prospectively. Of 101 patients studied, 45 developed rheumatic symptoms during the first six weeks after the operation. Thirty eight patients reported pain in the region of the shoulder girdle with no loss of shoulder function (postpericardiotomy rheumatism). Three of these patients also had features compatib...
